Photo and signature of the Applicant:- Applicant should upload his/her scan image; recent passport size photograph [File size not more than 50KB size and width up to 250 pixel and must be ".jpg" or ".jpeg" or ".png" format]..
Name of the Applicant: Applicant should write his/her name as given in academic certificate.
Contact Details:-Telephone Numbers should be properly mentioned (If landline number is filled-up by the applicant for alternate number). Applicant can be contacted through this mode also.
Mobile Numbers:- Applicant should properly mention his/her mobile phone numbers as it will be used for sending OTP and it will be the key to know the applicant.
Present Place of Residence:-Applicant should mention his/her Present Residence.
Present Place of Posting:-Applicant should mention his/her present posting.
Email Address:-Applicant must mention his/her Email ID properly; this will be the mode of communication.
Date of Birth:-Applicant Date of Birth will be in YYYY/MM/DD Format.
Date of Retirement:-Date will be in YYYY/MM/DD Format.
Nature of Employee:-Applicant should select his/her nature of Employee e.g. State , Central, PSU etc
Pay Scale:-Quarter will be allocated base on his/her Pay scale
EIN/MGEL number:-Applicant should mention his/her EIN(Employer Identification Number) or MGEL(Manipur Government Employees List) number
